Socially Determined
Socially Determined is a healthcare technology startup that has developed an analytics platform to quantify and visualize social determinant risk, down to the block level, using public and proprietary social determinants of health data.
Our investment in Socially Determined aligns with our focus on whole-person care and complements its focus on bringing forward solutions for its most vulnerable populations. The alignment will strengthen payer risk stratification and targeting efforts for population health initiatives.

FIGmd is a leader in provider healthcare solutions to connect, support, and consolidate clinical data. The company is working with both providers and payers to present healthcare information in a consumable and actionable fashion. Their clinical library continues to grow, with over 457M clinical records and 121M unique patients connected.
We invested in FIGmd with the belief that payers and providers will continue to require an exchange of clinical data. FIGmd has secured contracts with healthcare payers to aid in billing, HEDIS scoring, and closing gaps in care for members.
